How To Submit a FOIA Request

Your request must be in writing listing the information you are seeking. Provide as much information as possible in your request on the subject matter.

Your request must be addressed to one of the FOIA Officers.

Example:

FOIA Officer Erica Bailey
151 N. Morgan St
Shelbyville, IL 62565

Your letter should include your name, address, the date and the daytime phone number in case we have questions.
